Please read our office policies.
* We request that you provide your current health insurance card at every visit and inform the staff of any changes in your personal information.
* Patients are responsible for verifying with your insurance, that our provider participates in your insurance.
* Please be advised that if for any reason your insurance company will not reimburse your medical nutrition visit(s), then it will be the patient's responsibility to cover dietitian services at the discounted rate.
* Copayment is the responsibility of the patient at the time of the appointment.
* Please note that if required by your insurance it is your responsibility to obtain a referral or pre-authorization.
* There will be a $45.00 fee for missed appointments that are not cancelled at least 24hours in advance.
* There is a $30 charge for returned checks.
